Residential Program
The Residential Program of Perram House provides nursing and medical care for those who are terminally ill.  This clinical program offers expertise in the areas of mental health, addictions and palliative care.  The merger of these normally distinct areas of nursing practice has established a highly-skilled team that is comprehensive in its approach and responsive to the care complexities of each individual patient.  Our program goes beyond simple symptom management and control.

Efforts are made to alleviate suffering, facilitate the realization of particular goals and/or dreams, reconnect individuals with estranged family members, and provide support as patients face mortality, prepare for death and keep them comfortable while they die.

The Program encompasses:

Harm Reduction: A strategy toward patient care minimizing harm from prolonged use of addictive substances when treating pain and addressing symptom management in terminal illness.

Bereavement Care: The nursing staff engages in bereavement work with the loved ones of the patient as part of the overall program.

Mental Health: The care, offered in a unique home environment, includes an effort to empower the patient, build inner confidence so that they can develop inner strengths and minimize unresolved issues.  Social Workers, a psychologist and a psychiatrist are accessible to the resident and nursing staff within the clinical program. 

Spiritual Care:  available and is based on patient’s culture and faith

Caregiver Support: offered within the clinical program as early intervention to reduce the impact of mental illness (or the chronicity of mental illness) surrounding grief and bereavement.

Respite Care: This is palliative care relief to the caregiver who requires a break from home care of a loved one.  This reduces stress and avoids burnout of the caregiver while continuity of care is provided to the patient within the hospice.

Diabetic Care within Palliative Care: offered in residential and respite programs through stress reduction, motivation to exercise if possible, and the taking of appropriate nourishment. 

Teaching centre: Currently, both Ryerson University and Humber College have student placements in our preceptorship program.

Catchment Area: The population served is downtown Toronto

Eligibility: Adults over age 19 years with terminal illness (non-specific).  Priority is based on urgency and need.
