Perram House Hospice
Perram House is a Residential Hospice providing end-of life-care in Toronto. Patient-centric
palliative care is delivered to individuals who have a limited life expectancy.
Care is provided by an interdisciplinary team of care professionals, all of whom
specialize in palliative care. This program includes: nursing, personal support,
psychosocial and spiritual care placing the resident in control of her or his life.
Funding Model
There is no charge to residents for the services received.
Perram House gratefully acknowledges funds received from the Ministry of Health,
the Toronto Central LHIN through Toronto Central (CCAC) Community Care Access Centre
for care costs, including nursing and personal support worker salaries.
All other costs, including supplemental care costs, capital costs, operational costs
and administrative costs are funded through donations and fundraising activities.
We welcome your support. We are a community program funded by the community for
the community.
Our Goal
To offer end-of-life care to people living on their own in a home with holistic,
community based services, delivering patient-centric care, pain free to the extent
medically possible, while supporting their freedom to live life to the fullest.

Adults aged 19 or over with terminal illness (Nonspecific).

Frequently asked Questions
Q: What type of care is available?
A: Perram House has a team of healthcare professionals,
all of whom are specially trained in palliative care. This includes a Registered
Nurse (RN), and a Personal Support Worker (PSW) on site at all times (24 x7). Additional
nursing support is available during prime times as well as volunteers on a scheduled
basis. We also have a team of 5 palliative care doctors and a Medical Director available
to provide onsite visits and care.
Q: When should I contact Perram House?
A:
When no other alternatives are suitable for me and living independently by myself
is no longer possible.
Q: What is the admission process?
A: You can contact Perram House at any time during
normal business hours or leave a message after hours. We recommend familiarization
with our program and suggest that you or a family member visit the hospice to learn
more about our facility and the care services that we are able to provide. The Admission
Coordinator will advise you about next steps.
Q: Do I have to change doctors?
A: No. Your doctor may continue to be involved
in your care to the extent he/she is willing and able to do so.
Q: Can I continue my treatment?
A: Yes. Decisions about your treatment will continue
to be determined by you and your attending physician. Our care team will support
your program.
Q: Are there restrictions on friends and family
visits?
A: We encourage visits by friends and family members
(including pets), subject to normal considerations for others.
Q: What type of rooms are available?
A: Perram House has 8 private bedrooms available.
Q: Can people stay with a resident
overnight?
A: Yes. Every effort will be made to enable overnight
stays, including access to temporary beds where space and availability permit.
Q: What about meals and dietary restrictions?
A: Our team of cooks is able to support a wide
range of cultural tastes and dietary restrictions.
Q: How can I be admitted to Perram House?
A: We encourage you to contact our Admissions Coordinator
(416-925-9866 ext 228). No referral is required. Anyone can call-including you,
a friend or a family member. Alternatively you may ask the CCAC or your doctor's
office to inquire on your behalf.
Q: Will there be a cost to me?
A: No. There is no cost to you.
Q: May I go home for a weekend?
A: Yes. All residents are encouraged to live their
lives to the fullest, engage in the familiar routines of home and the activities
of their community to the fullest extent possible.
Q: May I go out to a movie or a walk?
A: Yes. Outings of this nature are encouraged,
as you are able. A volunteer may accompany you if required.
Q: Are other services available at Perram House?
A: Yes. Perram House offers the services of a Psychiatrist
and Psycho-therapist in addition to our team of medical physicians.
Q: Will I be alone?
A: No. You will not be alone in your time of need.
Members of your family, friends, our staff, and volunteers will give you the strength
support and companionship you want or need.
